Transcript Request Procedures (Woodson High School)

Transcript Assistant

Current Students

To request a transcript be sent to colleges, universities, scholarship sponsors, employers, or other similar organizations, you will need to complete, sign and return to Student Services the Consent For Release of Students Records in Support of Postsecondary Applications (IS-111) located here:  IS-111

Please submit this form before or with the Transcript Request Form located here: Transcript Request Form.  If the IS-111 is not submitted the Transcript Request Form cannot be processed.

Both the IS-111 and the Transcript Request form must be completed and signed by the student. If the student is under the age of 18, it must also be signed by his/her parent or guardian, and then submitted in person to Student Services or via email to @email, before any records can be released to a college, employer, or scholarship organization.

Transcript requests must be submitted at least 15 school days prior to the first college admission deadline (see deadline dates below). There is no charge for transcript requests for current students.

Seniors are reminded that their mid-year grades will automatically be sent to all colleges for which they have requested a transcript. A final transcript will be sent to the college designated on the spring Senior Survey.
