Current Students
To request a transcript be sent to colleges, universities, scholarship sponsors, employers, or other similar organizations, you will need to complete, sign and return to Student Services the Consent For Release of Students Records in Support of Postsecondary Applications (IS-111) located here: IS-111.
Please submit this form before or with the Transcript Request Form located here: Transcript Request Form. If the IS-111 is not submitted the Transcript Request Form cannot be processed.
Both the IS-111 and the Transcript Request form must be completed and signed by the student. If the student is under the age of 18, it must also be signed by his/her parent or guardian, and then submitted in person to Student Services or via email to @email before any records can be released to a college, employer, or scholarship organization.
Transcript requests must be submitted at least 15 school days prior to the first college admission deadline (see deadline dates below). There is no charge for transcript requests for current students.
Seniors are reminded that their mid-year grades will automatically be sent to all colleges for which they have requested a transcript. A final transcript will be sent to the college designated on the spring FCPS Senior Survey.
Transcript Request Deadline Dates
Please allow at least 15 school days prior to your first college application deadline for processing your transcript request. If your college admissions deadline is not listed below, the Transcript Request Form and IS-111 must be submitted to Student Services 15 school days before your first deadline.

This must also be submitted at least 15 school days prior to your application deadline. A letter will not be written without submission of this form. You can submit your application as soon as your parts are complete. Recommenders can/will submit letters independently.
Make sure you have invited your counselor and/or teachers as recommenders for the Common/Coalition Applications. Remember that even if your application is complete, colleges will likely not review it until the application deadline.
